Responsibilities:
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Consistently applies the corporate values of respect honesty and fairness and the constant pursuit of excellence in improving the health status of the people of the region through the provision of customer-friendly geographically accessible and high-value services within the environment of a comprehensive integrated academic health system. Is responsible for knowing and acting in accordance with the principles of the Lifespan Corporate Compliance Program and Code of Conduct.
Interviews patients and families to obtain pertinent information concerning patient and family needs in the context of family functioning.
Provides patient and family information to appropriate hospital personnel and community agencies in order to assist in resolving identified issues.
Develops a service plan and provides services to patient and family in accordance with the service plan.
Maintains appropriate documentation and statistics regarding case activity.
May participate in departmental studies and projects to include planning and coordinating activities.
Maintains list of hospital and community services and resources.
Maintains and enhances professional expertise through appropriate educational activities as necessary.
Assists in the resolution of practical problems of daily living and other concrete services including transportation housing education and training etc.
Arranges and coordinates connective referrals to appropriate community agencies and programs.
For social work coordinators who work in a psychiatric setting the following duties and responsibilities prevail:
Affects those discharge plans which call for nursing home placement assisted living facilities group homes chronic care facilities or other similar placements.
Carries out utilization review activities.
Maintains contact with managed care companies and programs to secure authorizations for admission and ongoing care.
